Medical Requirements for Athletics
All students participating in any of the athletic activities are required to have an annual sports physical examination.  Once a physical has been obtained and turned in, it will be valid for all activities in which the student participates during that school year.  Students must also have a signed insurance waiver and concussion policy agreement on file (see below).  The waiver is simply a verification that parents have adequate insurance coverage on the student.  The physical, insurance waiver and concussion policy agreement must be on file before the student will be permitted to try out for any athletic activities. Applicable fees must also be paid.  (New in 2021: A covid waiver form is also required.) Prior to a tryout, your child should pick up a gold “good to go” card from the office which indicates to the coach that all forms/fees have been completed. The gold card should be brought to the tryout.
